Below, please find PDFs of the forms you will need to read, complete, and send to the Graduate Institute Office. These forms are due for each semester as follows:

- US Dept. of Education Form - required for new students and first-time transfer students from Santa Fe
- Graduate Institute Health Form - required for new and first-time transfer students from Santa Fe (Others are encouraged to submit a new health form if their health status has changed.)
- Tuberculosis Form for All International Students - required for all international students attending St. John's College
- Students with Disabilities: Students who would like to request accommodations under the Americans with Disabilities Act or Section 504 of the Rehabilitation Act should submit the Disabilities Registration Form.
